Create basic Microsoft Form to capture employee self-assessment questions – Upwork


We have a skills matrix which contains the following:

22 Leadership attributes
33 Management skills (across 5 areas)
8 technical skills

and it is in an Excel format, which feeds into different proprietary charts. We cannot share the Excel file with employees.

We have determined the best way of collecting data from employees to put into this matrix is via a Microsoft Form, since we are based in the Microsoft ecosystem.

We are looking for someone who can create this form for us with a radio select button format for each question showing answer options 0 to 5. We will supply all the questions.

The form will need to include an information section explaining the different levels 1 – 5 and be branded to our business.
